Should I mention the person who died by name?
Yes, if you know it. A name shows the family you are mourning someone you knew, not just responding to news. In Arabic, check the form of the prayer: «غفر الله له» for a man and «غفر الله لها» for a woman. This page gives both forms, plus neutral ones that work in any case. If you do not know the name, a general prayer for the family is enough.
